Terms
Any material(s) or information provided on this web site is for information
purposes only. ShopLens cannot provide you with any specific advice in regards
to the wear, use, or care of contact lenses. You must check with your eye care
professional for any advice on contact lenses.
ShopLens is a contact
lens replacement company and as such, consumers who wish to purchase lenses from
ShopLens must demonstrate the availability of a valid and current medical
prescription issued by a licensed eye care professional.
ShopLens believes that you should have your eyes examined regularly and
always follow your Eye Care Professional’s instructions for the proper use and
care of your contact lenses. If you should have any discomfort at all with your
contact lenses, please discontinue use and contact your Eye Care Professional.
In order for ShopLens to sell you contact lenses you must ACCEPT THE
FOLLOWING TERMS:

You are successfully wearing contact lenses with a prescription that
matches the contact lenses you are ordering from ShopLens
-
You have a valid prescription for such contact lenses, issued within the
time frame required by law in your State or Country of residence obtained
through a physical eye examination conducted by a licensed eye care
professional.

You have not since your last prescription, been advised to stop wearing
contact lenses by an eye care professional or medical practitioner.

You are accepting that no responsibility or liability can be assumed by
ShopLens, its parent company or its affiliates, officers, directors, employees,
or agents, relating to your purchase of contact lenses from us.

You have read, understood and accept our Privacy Policy which states; ShopLens values its customers and respects their privacy and is committed
to safeguarding your privacy online. The customer information we collect, such
as your prescription information, email address, billing address, shipping
address, credit card, and Eye Care Provider is used only to process your order
and contact you in relation to your account and to communicate with you about
our products, services, contests, surveys, and promotions. The doctor’s
information we collect from you is to verify your prescription and ensure your
safety. The only communication ShopLens will have with your doctor is
verification of the prescription.

ShopLens may use your contact information to send you promotional material
and information about our company. When you register, you are given an
opportunity to opt-in to receive this material.
We don't disclose, sell,
rent, or loan any information about you to any third party, unless appropriate
in connection with the sale or transfer of all or part of the business. ShopLens
recognizes that it must maintain and use customer information responsibly.
To process your order we collect similar information that any e-commerce
website gathers to be able to fill your order. This information includes; your
name, email address, mailing address, shipping address, contact lens
prescription, phone number, credit card numbers, and the name and phone number
of your eye care provider. We also maintain a record of your historical
purchases that are placed through the phone, mail, fax, email, and over the
Internet to improve your overall shopping experience at ShopLens.
When you
make a purchase from ShopLens, we use your information to process your order. We
will also send you an email to confirm your order, and we may need to contact
you via phone, mail, or email if we have any questions regarding your order. In
addition we may use that information to help us improve our site design and your
shopping experience. If you make a purchase from ShopLens, we may also send you
notice about our products, services, contests, surveys, and promotions.
The
information you provide us over the Internet is transmitted over secured HTTPS
transmission using the strongest available encryption provided by Secure Sockets
Layer (SSL) to protect your privacy. In addition, any sensitive data that you
provide to us, such as, your password will be encrypted prior to storing it in
our database.
You are welcome to visit our site and review our policies any
time to be up to date on our latest revisions. If there is ever a change to our
policies, it is always posted on the site for your review.

You have read, understood and accept our Return Policy which states;
ShopLens accepts returns and exchanges for credit provided they fall within the
following criteria:
a)
The request for return happens within the 30 day period of the ship date.
b)
The product intended for return has not been opened or written on and is
in its original fully sealed manufacturer box or vial packaging.
c)
You must contact our Customer Service first at
CustomerService@ShopLens.com to
obtain a Return Merchandize Authorization number (RMA).
d)
Once you are issued an RMA# and given the return address for the package
then you can write the RMA # on the outside of the shipping box and ship the
package back. Please ensure proper and safe packaging for the lenses boxes.
e) Please ensure that you obtain a delivery confirmation from the shipper to
ensure that we receive your package. ShopLens is not responsible for lost or
damaged shipments.
Defective Product Replacement –
ShopLens will replace the
product upon the return of the defective product according to the following
steps:
a) The request for return happens within the 30 day period of the ship date.
b) You must contact our Customer Service first at
CustomerService@ShopLens.com to
obtain a Return Merchandize Authorization number (RMA).
c) A new (replacement) order will be processed and charged to your credit
card account.
d) Once you are issued an RMA# and given the return address for the package
then you can write the RMA # on the outside of the shipping box and ship the
package back. Please ensure proper and safe packaging for the lenses boxes.
e) Please ensure that you obtain a delivery confirmation from the shipper to
ensure that we receive your package. ShopLens is not responsible for lost or
damaged shipments. is received and verified as defective
by the manufacturer, a credit will be issued for the returned product to your
credit card account.
Exclusions from our Returns & Exchanges policy –
a) Any and all products that are custom made or made to order (MTO) can’t be
returned or exchanged.
b) Any product listed on our site as final sale, non-returnable or
non-exchangeable can’t be returned or exchanged.
c) Any product that has any writing on it or is visibly scratched or damaged
can not be returned or exchanged.
d) Any product that has been opened from its original manufacturer packaging
can’t be returned or exchanged. Please note; boxes or vials that have been
opened from their original outer package and seal is considered opened
and can not be returned.
e) Any claims made after 30 days from the ship date can not be honored.
Issuing of Credit –
a) A credit for the returned product will be issued to the credit card
account originally billed less the order shipping fee.
b) If the order qualified for complimentary shipping because it was over the
order minimum requirement and the return brings the order subtotal under the
minimum amount required, then a ground shipping fee will be imposed and deducted
from the credit to be issued.
c) The return of any portion of an order will trigger a recalculation of the
unit price billed and the order unit price will be matched up to the published
appropriate unit price level posted on the product website page.
You understand and agree that ShopLens fulfills its obligation by
processing your order and shipping it to the address destination you enter on
the order..

You understand and agree that it is your responsibility to verify your
order is filled correctly upon receiving the package and before you open the
contact lenses boxes. Any boxes opened can not be returned and credit can not be
issued for them.
