You are successfully wearing contact lenses with a prescription that matches the contact lenses you are ordering from ShopLens

You have a valid prescription for such contact lenses, issued within the time frame required by law in your State or Country of residence obtained through a physical eye examination conducted by a registered eye care professional licensed in your country, state, province or other applicable jurisdiction;

You have not since your last prescription, been advised to stop wearing contact lenses by an eye care professional or medical practitioner.

You are accepting that no responsibility or liability can be assumed by ShopLens or its affiliates, officers, directors, employees, or agents, relating to your purchase of contact lenses from us.
